26.5% of Emergency Room visits could be avoided if patients had better access to Community Healthcare Professionals.

This is the percentage of new publicly funded home care patients, of all ages, who had an unplanned emergency department visit in 30 days after leaving hospital. A lower percentage of patients is better. Timely follow-up after hospital discharge can help prevent the return of home care patients to the emergency department.
